Show your Eutectic pride, and help support student scholarships with University of Health Sciences and Pharmacy in St. Louis license plates! These plates are available only for registered Missouri vehicles and can be ordered at any time, regardless of your current license plate expiration date.

1. Complete the form below and make a contribution of either $25 (1-year) or $50 (2-year) or mail a check to Office of Advancement, University of Health Sciences & Pharmacy, 1 Pharmacy Place, St. Louis, MO 63110 and indicate L2L in the memo line. Your donation will help support the University’s scholarship program.

2. Once processed, your completed Emblem Use Authorization Form (EUAF) will be emailed to the email address you provided. (The Emblem Use Authorization Form is proof of your contribution to UHSP and will be required when you submit your application to the motor vehicle bureau.)

3. Complete a personalized license plate application. UHSP plates allow six spaces for your personalized message. (All UHSP plates are considered personalized and require a state personalization fee.) To see if your “message” is accepted, call the Missouri Department of Revenue (MDR) at 573.751.4509, or visit the MDR Web site to preview your personalized plate.

4. Take your application, emblem-use authorization form, and Missouri personalized plate fee to your local license bureau or mail to:

5. You will be notified by the Missouri Department of Revenue once your plates have been sent to the license office you indicate on the application.

* Note: The Motor Vehicle Bureau will only accept applications and renewals dated within six months of your license plate tags’ expiration. To ensure your application will be accepted, please do not submit your order prior to six months of your license plate expiration date.
